Fees, Insurance, Managed Care, Method of Payment, and Related Issues

Fees
Initial Evaluation: The initial evaluation is scheduled for 90 minutes, but the fee is $165, which will be charged to your credit card when the evaluation is scheduled. For your convenience, you may complete the New Client Information Form from this website and bring it with you to your first session.

Individual Psychotherapy Session (45 minutes) - $125.00

Couple Psychotherapy Session (45 minutes) - $125.00 I recommend that couples schedule
double sessions for a few times at the beginning of our work together then the length can be
reduced to a single session.

Insurance and Managed Care
To safe-guard the confidentiality of my clients, I prefer staying uninvolved with insurance companies; however, my staff will provide you with receipts which are suitable for filing for reimbursement. I am not on any managed care panels as I refuse to compromise the quality of care for my clients; however, some managed care plans allow for limited “out of network” benefits.

Exclusions
I do not work with children or adolescents, nor do I work with problems defined exclusively as drug or alcohol abuse.

Cancellations and Emergencies
Cancellations: Cancellations made less than 24 hours in advance will be charged at the usual session rates, unless there are extenuating circumstances. There is a twenty-four hour answering service for your convenience.

Emergencies: You are urged to go immediately to the nearest hospital emergency room or to call 911. Mr. Carroll is not able to assist in medical emergencies.

Scheduling
Each session is scheduled individually by Mr. Carroll. Since I know that your time is as valuable as mine, I work very hard to stay on schedule. If ever I am 15 minutes late for your appointment without notifying you earlier, your next session is on me.
